FUNCTIONAL INDEPENDENCE MEASURE (FIM) AND FUNCTIONAL INDEPENDENCE MEASURE FOR CHILDREN (WeeFIM)
The Functional Independence Measure (FIM) is a tool designed to evaluate the functional abilities of patients during the course of their rehabilitation following conditions such as stroke, traumatic brain injury, spinal cord injury as well as many other conditions. The FIM is an 18-item measure which seeks to gauge an individual’s functional capacity across the domains of self-care, continuance, mobility, transfers, communication and cognition. For children, the WeeFIM tool can be used instead.
The FIM can be carried out on its own or as part of a more comprehensive occupational therapy assessment such as a functional capacity assessment (FCA). Each item is scored on a 7-point scale, with higher scores indicating greater levels of independence. The overall FIM score assists in determining the individual's level of disability, required assistance and progress over time.
Functional Independence Measure (FIM):
The FIM is a standardised assessment tool used to evaluate the functional independence of adults in performing activities of daily living. The FIM measures 18 items across six domains:
- Self-Care: Eating, grooming, bathing, dressing (upper and lower body), toileting.
- Sphincter Control: Bladder and bowel management.
- Mobility: Transfers (bed/chair/wheelchair, toilet, tub/shower).
- Locomotion: Walking, wheelchair use, stairs.
- Communication: Comprehension and expression.
- Social Cognition: Social interaction, problem-solving, memory.
Paediatric Functional Independence Measure (WeeFIM):
WeeFIM is an adaptation of the FIM designed specifically for children aged 6 months to 7 years, or older children with developmental delays. WeeFIM measures the child’s functional independence in daily activities within three domains:
- Self-Care: Eating, grooming, bathing, dressing (upper and lower body), toileting.
- Mobility: Transfers (bed/chair/wheelchair, toilet, tub/shower), locomotion (crawling, walking, wheelchair use, climbing stairs).
- Cognition: Comprehension, expression, social interaction, problem-solving, memory.
The WeeFIM, like the FIM, uses a 7-point scale to rate performance, facilitating the development of individualised intervention plans and tracking of progress.
